Always cut a hole in the inside bottom of a closed bottom crown. This hole can be cut with a drill bit or a round bur; moreover, this will allow clearance for the culet of the stone. If this clearance is not provided, the stone may chip during setting, or while wearing if the customer knocks the stone.

Great advice Brad. I also find that it is much easier to clean a piece with the opening behind the stone.

After I've cut the hole, I finish the edge by chamfering the rough edge with whatever burr will fit into area. My first choice is a 'setting burr' since it has a fine edge. Then, I use rubber wheels and polish to complete the job.
